On August 7, 2007 the KP Santa Clara hospital at 900 Kiely Blvd will move to the new medical center at the corner of Homestead Rd and Lawrence Expressway.

All Hospital and Emergency Services will be available at the new hospital, at 700 Lawrence Expressway, from 7a.m on August 7, 2007.

If you are visiting the medical center on August 7, please use the Lehigh Drive entrance to the facility on Lawrence Expressway. The Homestead Rd entrance to the campus will be closed to patient, visitors and employees at 7a.m and will be used exclusively for ambulances moving patients from the Kiely hospital to the new hospital.

Please note: The move of the hospital and emergency department does not affect the existing primary and outpatient services that we offer here at Mountain View medical offices. If you normally receive routine care here, you can continue to do so.

Be prepared with an Advance Health Care Directive

To prepare for the unexpected, all adults-regardless of age or health status-should discuss their wishes with their family and physician, and complete an Advance Health Care Directive.

In the event of a serious illness or injury, you may not be able to make important medical decisions for yourself. This legal document allows you to do two things:

  1. Appoint another person to be your health care "agent" or "attorney-in-fact." This person will have legal authority to make decisions about your medical care in you become unable to make these decisions yourself.
  2. Write down your health care wishes. For example, you can decide whether or not to receive treatment that prolongs the dying process if you are terminally ill. Your physician wand your agent must follow your lawful instructions.

For a free Advance Health Care Directive Kit, call our Member Service Call Center at 1-800-464-4000 (English), 1-800-788-0616 (Spanish), 1-800-757-7585 (Chinese dialects), or 1-800-777-1370 (TTY), seven days a week, from 7 AM to 7 PM. The kit includes forms and instructions for completing them.
